Must reside in Summit County, UT. Must meet WIC income guidelines. Must have a nutritional or medical risk. For pregnant, breastfeeding, and postpartum women, infants and children up to age 5.
Important Information
Appointments preferred. Closed the first Monday through Friday of the month and the last Friday of the month.
