Must be Indiana resident; must be categorically eligible (pregnant women, breastfeeding women up to 1 year, postpartum women up to 6 months, infants up to 1 year, children up to 5 years); must be at nutritional risk; must be income eligible (automatic eligibility if receiving Medicaid, TANF, or Food Stamps/SNAP)
